Job Description

  • Perform professional nursing, consultation, and educational services for persons with diabetes, and provides community health education to achieve the company’s goals and objectives of the business
  • Educating /counselling of new and known Diabetes patients
  • Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection
  • Educate clients on how to use the Accu-chek glucometer for self-monitoring
  • Liaise with team to organise diabetes events such as outreaches and support group
  • Planning and coordination of ISN Chronic Care clinic
  • Visit diabetes clinics and attend diabetes association meetings
  • Coordinate and implement the company’s vaccination programs such as hepatitis.

Education and Work Experience:

  • Minimum of a Bachelor of Science in Nursing. Certification as a diabetes educator is an advantage
  • Minimum of two years’ experience in providing diabetes education to patients.

Competencies Required:

  • In-depth knowledge about current diabetes treatment management
  • Good organization skills and the ability to handle multiple tasks are required
  • Demonstrates working knowledge of diabetes management of patients with diabetes and diabetes complications
  • Demonstrates the ability to effectively communicate and provide patient education
  • Good communication and interpersonal skills.
